Hot flushes at 31. Periods that come every two months, then every four. Doctors who say “it’s stress”. This is how many women with premature ovarian insufficiency begin their story.
What POI Is
Premature ovarian insufficiency (POI) is when the ovaries stop working normally before the age of 40. Unlike menopause, it is not always permanent or complete — ovarian activity can fluctuate, and some women with POI still ovulate occasionally. The NHS covers it under early menopause.
Signs
- Irregular or missed periods before 40
- Hot flushes and night sweats
- Vaginal dryness and discomfort during sex
- Low mood, poor concentration, poor sleep
- Reduced sex drive
Causes
Often, no cause is found. Known causes include genetic factors, autoimmune conditions (including thyroid disease), previous chemotherapy or radiotherapy, and surgery on the ovaries.
How It Is Diagnosed
Usually through raised FSH levels on two blood tests several weeks apart, together with symptoms. AMH is often very low — though, as we explain in Low AMH Does Not Mean You Cannot Conceive, a single number never tells the whole story.
Why Early Diagnosis Matters Beyond Fertility
Oestrogen protects the bones, heart and brain. Women with POI are usually advised to take hormone replacement until the natural age of menopause to protect their long-term health. This is important and should not be skipped.
What About Pregnancy?
We will be honest: POI makes natural conception difficult. But because ovarian activity can return intermittently, a small proportion of women do conceive. Others build their families through donor eggs. Every path deserves support.
Our role is to support your overall hormonal health, your emotional wellbeing, and — where ovaries are still showing some activity — to give every cycle the best possible conditions. If your periods have become erratic before 40, please do not wait.
