Milk in Your Breasts but No Baby — What High Prolactin Is Telling You

A woman notices a drop of milky fluid from her breast. She is not pregnant. She is not breastfeeding. She has not been for years — or never has.

Most women tell no one. They assume it is shameful, or imaginary, or a bad sign. It is none of those things. It is a signal, it has a name, and it is one of the more treatable causes of infertility we see.

What Prolactin Does

Prolactin is the hormone that produces breast milk. It rises in pregnancy and stays high while breastfeeding — and while it is high, it deliberately suppresses ovulation. That is the body spacing pregnancies, and it is exactly as designed.

The problem comes when prolactin is high and you are not breastfeeding. The suppression still happens. Ovulation slows or stops, cycles become irregular or vanish, and conception cannot occur — regardless of how carefully anyone times anything.

The Signs

  • Milky discharge from one or both breasts (galactorrhoea) — sometimes only when pressed
  • Irregular periods, very light periods, or no periods at all
  • Difficulty conceiving
  • Reduced libido
  • Vaginal dryness
  • Headaches or changes in vision — this combination needs prompt medical attention

Note that you can have raised prolactin with no breast discharge at all. Absent or irregular periods alone are reason enough to have it checked.

Why Prolactin Rises

  • An underactive thyroid. One of the most common and most missed causes — low thyroid hormone drives prolactin up. Which is why these two are always tested together. See The Small Gland That Can Stall Your Fertility.
  • A prolactinoma. A small benign growth on the pituitary gland. The word “tumour” frightens people, but the Mayo Clinic notes these are non-cancerous and usually respond very well to medication rather than surgery.
  • Medications. Some antipsychotics, antidepressants, anti-nausea drugs and blood pressure medicines raise prolactin. Bring your full medication list to any fertility appointment.
  • Chronic stress and poor sleep. Both nudge prolactin upward — see Stress, Sleep and Your Fertility.
  • Persistent nipple stimulation or chest wall irritation.
  • PCOS — mildly raised levels are sometimes seen alongside it.

Getting the Test Right

Prolactin is easily thrown off by how the sample is taken, and a falsely high result causes needless alarm. To get a reliable reading:

  1. Test in the morning, a few hours after waking
  2. Avoid breast or nipple stimulation, and sex, the day before
  3. Rest quietly for around 20–30 minutes before the blood is drawn — stress alone raises the level
  4. Have a single mildly raised result repeated before anyone acts on it

If levels are genuinely and persistently high, an MRI of the pituitary may be arranged to look for a prolactinoma. That is thoroughness, not bad news.

The Hopeful Part

This is one of the cases where the story often ends well and quickly. Correct an underactive thyroid, and prolactin frequently normalises on its own. Treat a prolactinoma with medication, and levels usually fall — with ovulation returning and pregnancy following for many women. Change a medication, where it is safe to do so with your doctor, and the same can happen.

The tragedy is only in the silence. Women who never mention the discharge because they are embarrassed can spend years being investigated for everything except the one hormone that would have explained it. Please say it out loud — to your doctor, or to us.
